Physician Services
​
If you do not have a Primary care Physician we will find one that suits your needs, listens to you, and that you like. Our doctors will visit you at home or we have doctors who do office visits only.If you have a Primary care Physician and you would like to keep your doctor we will work with him or her to get you the in-home help you and your family need. We also will provide rides to and from your primary care doctor; if it is the one we help you get or if it is the doctor you already had!! The doctor will set up a plan to get you healthy and to keep you that way. The doctor will also help you find any specialist doctors you need. We will work hard to find the right doctor for you!!
Nursing Visits

A Registered Nurse will visit after the doctor and she will coordinate all care, supervises the ancillary staff and is on-call for problems, should they arise. We will then have an LPN visit you at home at least once a week on any day and time that fits your schedule. The nurse will work with your doctor to get you healthy and keep you that way. She will discuss the plan your doctor set up with you and what needs to be done to follow it. She will also do things such as take your blood pressure, pain/medication management, chronic illness management(back, hip & joint pain), help with diabetic teaching and management (insulin, foot care, proper nutrition), and whatever else you need her do to better your health. We also have specialty nurses that help with health concerns like wound care, injections, colostomy care, and catheter care.
Specialist Services

We offer other services at home to help you or your loved one live healthy also like CNAs' for bathing and personal care, Medical Social Workers to help with Medicaid, food stamps, and other services, Dietitians to help plan a diet that will benefit your health, Speech therapists to help with swallowing, speaking & language (problems with retrieving words), and more. Lab work and Xrays ordered by your doctor will also be done at home! PCA's will come to help with light cleaning, personal care, and errands Plus anything else you think you need help with please feel free to let us know so we can help you get it.
